Please complete the online admission form once booking is confirmed and submit before the day of admission. One form is required for each cat. PLEASE NOTE THIS IS NOT A BOOKING FORM-contact the clinic on (03)95964804 to make a booking

Cats are required to have been vaccinated within 12 months prior to boarding, proof of vaccination is required on admission

Cats are required to be up to date with flea and worming control prior to boarding, or we can do on admission at owner expense, please advise below

Bedding and personal effects: we strongly request and recommend for belongings not to be brought in, to avoid risk of loss or damage.

Cat Carriers: we request,where possible,for carriers not to be left due to limited storage. If carrier must be left, please detail desciption below 

These may incur a fee- please enquire directly. Note nail clipping incurs a fee of $35
